As a volunteer, intern, fellow, or independent contractor supporting Second Harvest Food Bank of Santa Clara and San Mateo Counties (“SHFB”), I may be exposed to sensitive information. I understand that, during the course of my involvement with SHFB assisting in Food Connection, Cal Fresh outreach, or other SHFB departments or programs, I may have access to sensitive personal and private information about individual clients and their families, and about donors. Such information includes, but is not limited to, names, addresses, social security numbers, incomes, personal situations, and credit card information.
I understand and share SHFB’s commitment to individual and family privacy, dignity, and respect. At all times during and after my participation in SHFB programs, I will keep any information I learn when working at SHFB confidential, except as may be required in my role as a volunteer, intern, fellow, or independent contractor, or as expressly authorized in writing by the Chief Executive Officer of SHFB. Confidential information includes all information obtained in the course of my relationship with SHFB, whether through written records, both physical and electronic, or personal interaction.
I will store or dispose of records in accordance with SHFB policy, as directed by my supervisor. At the end of my participation as a volunteer, intern, fellow, or independent contractor, or upon SHFB’s request at any other time, I will deliver to SHFB all of SHFB’s property, equipment, and documents, together with all copies, regardless of whether such property contains confidential information.
I understand that violation of this confidentiality agreement may subject me to immediate termination of my assignment as a volunteer, intern, fellow, or independent contractor and any other civil and equitable remedies SHFB may have.
