Court Ordered Volunteer Application

Court ordered volunteers need to be pre-screened and approved before registering and starting their volunteer service. Hours that are performed without approval will not count. If you have any questions, please contact Volunteer Engagement at 408-266-8866 ext. 150.

If you do not need to complete court mandated hours, please register for a volunteer shift at SHFB.org




















































Volunteer Agreement

As a volunteer, I will:
  • Treat others with respect and dignity, refraining from engaging in harassing or inappropriate language and behavior
  • Maintain confidentiality in case I come in to contact with personal or sensitive information while volunteering
  • Provide an emergency contact name and phone number to the food bank, and identify myself with a volunteer name badge
  • Follow instructions from staff or volunteer leader
  • Report hazards or unsafe activities, and refrain from engaging in those activities myself including running, shouting, or horseplay
  • Use proper judgment when handling food. If I have a sensitivity to certain foods, I will avoid handling those products and notify supervisors.
  • Wear sensible, modest clothing
  • Not volunteer under the influence of alcohol or drugs, and only smoke in permitted areas
  • Not volunteer when ill, follow proper hand-washing techniques, and maintain proper hygiene
  • Not take food, other product, or accept monetary gifts.  I understand that unauthorized possession will be considered stealing and will lead to discontinuation of volunteering.
  • Understand that there is no mileage reimbursement or other financial incentive for volunteering
  • Be safe!  I will only do as much physical activity as I am comfortable doing, will use proper lifting techniques, and ask if I need assistance
  • Provide a parental permission slip if under the age of 18
  • Exercise good judgment
  • Report any behavior that is hurtful such as bad language, violence and unwanted comments to food bank staff

As a volunteer, I may be exposed to confidential information including sensitive personal and private information about individual clients and their families, and about donors. I understand and share Second Harvest’s commitment to individual and family privacy, dignity, and respect. At all times during and after my volunteer assignment, I will keep any information I learn when working at Second Harvest confidential. I will store or dispose of records in accordance with Second Harvest policy, as directed by my supervisor. At the end of my participation as a volunteer, or upon Second Harvest’s request at any other time, I will deliver to Second Harvest all of Second Harvest’s property, equipment, and documents, together with all copies, regardless of whether such property contains confidential information.


Confidentiality Agreement

As a volunteer, intern, fellow, or independent contractor supporting Second Harvest of Silicon Valley, I may be exposed to sensitive information. I understand that, during the course of my involvement with Second Harvest assisting in Food Connection, Cal Fresh outreach, or other Second Harvest departments or programs, I may have access to sensitive personal and private information about individual clients and their families, and about donors. Such information includes, but is not limited to, names, addresses, social security numbers, incomes, personal situations, and credit card information.

I understand and share Second Harvest’s commitment to individual and family privacy, dignity, and respect. At all times during and after my participation in Second Harvest programs, I will keep any information I learn when working at Second Harvest confidential, except as may be required in my role as a volunteer, intern, fellow, or independent contractor, or as expressly authorized in writing by the Chief Executive Officer of Second Harvest. Confidential information includes all information obtained in the course of my relationship with Second Harvest, whether through written records, both physical and electronic, or personal interaction.

I will store or dispose of records in accordance with Second Harvest policy, as directed by my supervisor. At the end of my participation as a volunteer, intern, fellow, or independent contractor, or upon Second Harvest’s request at any other time, I will deliver to Second Harvest all of Second Harvest’s property, equipment, and documents, together with all copies, regardless of whether such property contains confidential information.

I understand that violation of this confidentiality agreement may subject me to immediate termination of my assignment as a volunteer, intern, fellow, or independent contractor and any other civil and equitable remedies Second Harvest may have.

Warehouse Food Sorting/Food Distribution Volunteer Agreement

As a volunteer, I will:
  • Wear closed toe shoes and long pants, and NOT wear sandals, sleeveless shirts, or shorts, or otherwise revealing or offensive clothing.
  • NOT use ear buds in the sort room
  • Step out of the sort room to use my mobile device.  I may use my cell phone to take pictures with verbal permission of others.
  • NOT use unauthorized warehouse equipment, including forklifts and electric pallet jacks.
  • Leave valuables, including purses, backpacks, computers, etc. out of sight in my vehicle, OR utilize a food bank locker if available.  I understand that Second Harvest Food Bank assumes no responsibility for damage to or loss of personal property.
  • Assume responsibility for getting a staff signature at the end of my shift if I need to document volunteer hours for school or court.
  • Accept the Second Harvest policy on volunteer time keeping (ATTACHED)
  • Report any behavior that is hurtful such as bad language, violence and unwanted comments to food bank staff
  • Inform the Site/Team Leader if I am not feeling well during the distribution, or need to leave early for other reasons
